# High Blood Pressure: A Silent Threat to the Heart, Kidneys, and Eyes

High blood pressure, medically known as hypertension, is one of the most common chronic diseases worldwide. Its danger does not lie only in the high number seen on a blood pressure monitor, but in its silent and continuous effect on the blood vessels, heart, brain, kidneys, and eyes.

Many patients may live for years with high blood pressure without experiencing any obvious symptoms. There may be no headache, no dizziness, and no chest pain. However, damage may already have started inside the small arteries of the kidneys, the delicate vessels of the retina, or the heart muscle. This is why hypertension is often called “the silent killer.”

## What Is Blood Pressure? Blood pressure is the force exerted by circulating blood against the walls of the arteries as the heart pumps blood throughout the body. When blood pressure is measured, two main numbers are seen: Systolic blood pressure: This is the upper number and represents the pressure inside the arteries when the heart contracts. Diastolic blood pressure: This is the lower number and represents the pressure inside the arteries when the heart relaxes. For example, if the blood pressure is 140/90 mmHg, the number 140 represents systolic pressure, while 90 represents diastolic pressure. When blood pressure remains high for a long time, blood vessels gradually lose their elasticity, and microscopic damage begins in the inner lining of the arteries. Over time, this damage may lead to hardening of the arteries, enlargement of the heart muscle, impaired kidney function, and problems in the retinal vessels of the eye. ## Why Is High Blood Pressure Dangerous? High blood pressure is not merely a high number. It is a condition that affects the entire vascular system of the body. If left uncontrolled, it may increase the risk of stroke, heart attack, heart failure, abnormal heart rhythms, kidney failure, vision problems, bleeding or blockage in the eye vessels, and atherosclerosis. Large clinical studies, such as the SPRINT trial published in the New England Journal of Medicine, have shown that careful blood pressure control in selected high-risk patients may reduce the risk of cardiovascular disease and death. Therefore, the goal of hypertension treatment is not only to lower the number, but to protect vital organs in the long term. ## How Does High Blood Pressure Affect the Kidneys? The kidneys are among the organs most affected by high blood pressure because they contain a very delicate network of small blood vessels. Inside each kidney, there are millions of tiny filtering units called glomeruli. When blood pressure remains consistently high, this pressure is transmitted to the small vessels inside the kidneys. Over time, the excessive pressure begins to damage the walls of these vessels and the glomeruli. In the early stages, small amounts of protein may start leaking into the urine, especially albumin. This condition is called albuminuria, and it is an early sign that high blood pressure has started to affect the kidneys. If high blood pressure continues, scarring may develop inside the glomeruli. This condition is known as glomerulosclerosis. As a result, the kidneys lose part of their filtering ability. Creatinine levels may rise, the estimated glomerular filtration rate, known as eGFR, may decrease, and chronic kidney disease may develop. For this reason, a patient with hypertension does not only need blood pressure measurement. Regular kidney follow-up is also important and may include: Creatinine level. eGFR calculation. Urine analysis. Urine albumin-to-creatinine ratio. ## How Does High Blood Pressure Affect the Eyes? The eye, especially the retina, contains very delicate blood vessels. What makes the eye unique is that these vessels can be directly observed during a fundus examination. For this reason, the retina is considered an important window into the effects of high blood pressure on the blood vessels. When blood pressure rises, the retinal vessels initially try to protect themselves by narrowing. However, if high blood pressure continues for a long time, the vessel walls become thicker and stiffer, and the blood and oxygen supply to the retina may decrease. This may lead to: Narrowing of the retinal arteries. Bleeding inside the retina. Fatty deposits. Retinal swelling. Cotton-wool spots caused by reduced blood supply. Swelling of the optic nerve in severe cases. The basic mechanism is that high pressure damages the inner lining of the blood vessels and disrupts what is called the blood-retinal barrier. When this barrier is damaged, fluid, fats, and blood may leak into the retinal tissue, which can affect vision. Therefore, patients with high blood pressure, especially those with poorly controlled or long-standing hypertension, are advised to have a regular fundus examination. ## Salt and Blood Pressure: A Small Habit With a Major Effect Salt is one of the most important dietary factors associated with high blood pressure. The World Health Organization recommends that daily sodium intake should be less than 2000 mg, which is approximately equal to less than 5 grams of salt per day. The problem is that salt does not come only from the salt we add to food. A large amount of salt is hidden in everyday foods such as: Bread. Cheese. Olives. Pickles. Processed meats. Instant soups. Potato chips. Sauces. Fast food. Salt raises blood pressure by increasing fluid retention in the body, which increases the volume of blood inside the vessels. Over time, it may also impair the function of the arterial lining and increase vascular stiffness. A recent study published in JAMA in 2023 showed that reducing dietary sodium for only one week led to a clear reduction in systolic blood pressure. This confirms that salt reduction is not merely general advice, but a real therapeutic intervention that can produce measurable results. ## The DASH Diet: One of the Strongest Dietary Approaches for Hypertension One of the most scientifically studied dietary patterns for lowering blood pressure is the DASH Diet, which stands for: Dietary Approaches to Stop Hypertension This diet does not depend on a specific product or a temporary program. Rather, it is based on a healthy long-term eating pattern. The DASH diet encourages increased consumption of: Vegetables. Fruits. Whole grains. Legumes. Nuts. Fish. Chicken. Low-fat dairy products. At the same time, it recommends reducing: Salt. Saturated fats. Processed meats. Sugar. Sugary drinks. Fast food. An important advantage of the DASH diet is that it does not only help lower blood pressure, but also supports overall heart and vascular health. ## Practical Dietary Advice for Patients With High Blood Pressure Controlling blood pressure does not always require difficult changes. Sometimes, small and consistent changes are more beneficial than strict temporary diets. The most important recommendations include: Do not keep a salt shaker on the dining table. Do not add salt before tasting your food. Read nutrition labels on packaged products and check the sodium content. Reduce pickles, salty olives, and salty cheeses. Choose fresh foods instead of canned or processed foods. Reduce bread intake if consumed in large amounts. Increase vegetables, fruits, and legumes. Replace fast food with simple homemade meals. Use lemon, garlic, vinegar, herbs, and natural spices instead of salt. Foods rich in potassium, such as leafy green vegetables, legumes, yogurt, avocado, banana, potatoes, and lentils, may also be beneficial. However, patients with kidney disease should not increase potassium intake without consulting their doctor. ## Lifestyle and Its Role in Treating High Blood Pressure Medications are very important in the treatment of hypertension, but they are not the whole treatment. Lifestyle is a fundamental part of blood pressure control. Weight control: Excess weight, especially abdominal fat, is directly associated with high blood pressure. Regular walking: Brisk walking or moderate exercise for 150 minutes per week can help improve vascular flexibility and reduce blood pressure. Good sleep: Poor sleep and sleep apnea may contribute to persistent high blood pressure. If a patient has loud snoring, daytime sleepiness, or morning headaches, sleep apnea should be considered. Smoking cessation: Smoking damages the lining of the arteries and increases the risk of blood clots and heart disease. Stress reduction: Chronic psychological stress may increase the activity of the sympathetic nervous system, contributing to higher blood pressure. Habits such as walking, breathing exercises, prayer, meditation, and proper sleep regulation may help reduce stress. ## How Should Blood Pressure Be Measured at Home? Home blood pressure measurement is very important, but it must be done correctly. To obtain an accurate reading: Sit quietly for 5 minutes before measurement. Do not drink coffee or smoke for 30 minutes before measurement. Keep your arm at heart level. Sit with your back supported and your feet flat on the floor. Do not talk during the measurement. Measure at consistent times. Do not rely on a single reading; the average of several readings over a few days is more reliable. If blood pressure is very high, such as 180/120 mmHg or higher, especially with chest pain, shortness of breath, speech difficulty, weakness in one side of the body, blurred vision, or severe headache, emergency medical care should be sought immediately. ## When Should You See a Doctor? A doctor should be consulted if any of the following are present: Repeated high blood pressure readings. Headache associated with high blood pressure. Chest pain. Shortness of breath. Palpitations. Blurred vision. Swelling in the legs. Foamy urine. Elevated creatinine. Diabetes. Kidney disease. Family history of early heart disease. Hypertension treatment should be personalized. Not every patient needs the same medication, and not every patient has the same treatment target. Age, heart disease, diabetes, kidney disease, history of stroke, and current medications must all be taken into consideration. ## Conclusion High blood pressure is a silent disease, but it is not a simple one. A patient may feel completely well while the blood vessels, kidneys, eyes, and heart are under continuous pressure. Controlling blood pressure means protecting the heart from failure and heart attacks, protecting the brain from stroke, protecting the kidneys from deterioration, and protecting the eyes from retinal complications. Reducing salt intake, following the DASH diet, walking regularly, controlling body weight, sleeping well, avoiding smoking, and maintaining regular medical follow-up are key steps in prevention and treatment.
